The J.P. Farley Corporation provides a variety of Integrated Plans for our Plan
Sponsors. Our team of quality-certified experts is very familiar with
addressing customers’ unique needs.
Medical
Top
Medical Plan Coverage is designed to provide coverage for medical services as
well as protection against catastrophic health problems. The J.P. Farley
Corporation provides customized benefit solutions for Plan Sponsors wishing to
provide quality medical care for their employees.
Dental
Top
Dental plans are designed to help pay for the costs of preventative care and
other dental services. Several options are available including traditional
indemnity programs, PPOs, dual choice programs, and voluntary plan options are
available to Plan Sponsors.
Vision
Top
Vision plans are designed to help pay for the costs of preventative care and
other vision services. Several options are available including Self-funded and
Member Pay Options.
Prescription
Top
Prescription Benefit Coverage is designed to provide coverage for retail and
mail order prescriptions. The J.P. Farley Corporation has contracted with
several national vendors, including MedImpact, CareMark and ExpressScripts to
bring value to the Plan Sponsor.
Short Term Disability
Top
Short Term Disability plans are designed to provide partial income replacement
to employees of companies who fund this type of benefit, if they become
disabled due to sickness or injury usually 13 or 26 weeks.
Long Term Disability
Top
Long-term disability plans are designed to provide partial income replacement
to if an employee cannot work due to a non-work related illness or injury.
Long-term disability picks up where short-term disability leaves off, usually
paying a percentage of salary..
Life
Top
Life Insurance is a benefit plan that pays money upon a participant’s death to
their beneficiaries. Life insurance helps protect employees and their families
from financial hardship related to an untimely death. This can be in addition
to employer paid life coverage, additional amounts for the employee and/or for
their spouse and dependents.
Flexible Spending Accounts (Section 125 and 129 Plans)

Flexible spending accounts allow you to set aside money from your paycheck on a
before-tax basis to pay for allowable medical, dental or eligible dependent
care expenses. Flexible spending accounts (FSA’s) can be established for
medical expenditures: Medical Plan Premium Payments: Pre-Tax Premium Plan,
Medical Expense Reimbursements: Health Care Flexible Savings Account (HCFSA),
Dependent Care Reimbursements: Dependent Care Flexible Savings Account (DCFSA).
Health Reimbursment Accounts (HRAs)

A health reimbursement arrangement (HRA) must be funded solely by an employer.
The contribution cannot be paid through a voluntary salary reduction agreement
on the part of an employee. Employees are reimbursed tax free for qualified
medical expenses up to a maximum dollar amount for a coverage period. An HRA
may be offered with other health plans, including FSAs.
Health Savings Accounts (HSAs)

The Medicare Prescription Drug Act did much more than just provide coverage for
seniors for prescription drugs, it also has provisions that encourage people to
save for their own healthcare. Individuals are permitted to contribute up to
100% of their health plan deductible. The maximum annual contribution, indexed
annually, is $2,650 for individuals and $5,250 for family policies in 2005.
Care Management

Connected Care Services was initiated in 1988 by The J.P. Farley Corporation to
assist plan sponsors in providing integrated management services. Connected
Care Services provides the full spectrum of medical management services,
including case management, disease management and utilization review. The
objective of medical management is twofold: 1) Quality Assurance for
Health plans in the provision of services to the plan members is important
because quality is the best cost-effectiveness tool available to the plan and
its members, 2) Quality Assurance for Health Plans and Medical Choices for
Plan Participants Understanding medical choices and decisions are equally
important to plan members in feeling secure that the medical care they receive
is necessary for their condition.

Health Promotion Management focuses on promoting proper health care usage and
individual well-being. Tools are provided for the prevention, awareness, early
detection and lifestyle choices that assist and encourage an improved quality
and increased quantity of life. Our tailored, comprehensive approach addresses
