Verification / Faxback

In order to best serve our provider partners we require providers to obtain a Faxback before calling into our Provider Services Department. This allows us to serve providers more quickly and accurately. Our system will allow you to easily verify member eligibility, check the status of a claim, initiate an authorization, provide clinical follow up for a precertification and/or follow-up on a benefit or claims status.

In most cases the information obtained from our automated system will answer the questions that you have. If after obtaining a faxback you still have questions you will need to call our provider services department with this documentation in hand.

The Faxback system will enable you to:

  • verify member or dependent eligibility
  • check the status of a claim
  • provide information for authorization
  • provide clinical follow up on a precertification
  • follow up on benefits or claims status

Directions for obtaining a Faxback:

  1. Dial 800.634.0173 or 440.250.4300, Select Option 1
    to access Provider specific options.
    Follow the telephone prompts and input the appropriate information.*
  2. Press 1 to Check Member Eligibility or Press 2 Check on the Status of a Claim.
    You will be transfered to the faxback system for the remaining prompts.
  3. Press 1 to Check Member Eligibility or
    Press 2 to Check on the Status of a Claim

          Check Member Eligibility

  1. Enter your provider type:
    Press 1 if you are a Medical Provider
    Press 2 if you are a Dental Provider
    Presss 3 if you are a Vision Provider
  2. Enter your 9-digit TIN followed by "#".
  3. Enter the Plan Member's ID number** followed by "#".
  4. Enter the Plan Member's Date of Birth (MMDDYYYY) followed by "#".
  5. Press 8 to enter additional requests or
    Press * to enter the fax number (XXX-XXX-XXXX)

          Check on the Status of a Claim

  1. Enter your 9-digit TIN followed by "#".
  2. Enter the Plan Member's ID number** followed by "#".
  3. Enter the Plan Member's Date of Birth (MMDDYYYY) followed by "#".
  4. Press 1 to enter a single date of service  (MMDDYYYY)
    Press 2 to enter a date of service and charge amount
    Press 3 to enter a range of service dates (from MMDDYYYY thru MMDDYYYY)
  5. Press 8 to enter additional requests or
    Press * to enter the fax number (XXX-XXX-XXXX)

* Do to our recent faxback conversion our system is being updated to accept either the participant's 11-digit or 9-digit subscriber ID number or the 9-digit social security number.  The system will attempt to match dependents or spouses on the employee's social security number if the dependent's or spouse's social secuirty numbers are not in our system. ***

* If a provider is unable to access a member's information through our automated faxback system because the member's plan is new to J.P. Farley they may provide our representative with the member's group name and number in order to receive benefits through our verification system.

Please note that in all situations benefits will not be quoted over the phone.
